In a large bowl, combine ground chicken, breadcrumbs, Parmesan, mozzarella, garlic, egg, milk, parsley, Italian seasoning, salt, and pepper. Mix until just combined.

Shape the mixture into a loaf on the prepared baking sheet (or press into the loaf pan). Bake for 35-40 minutes until cooked through and firm.

Remove from the oven and sprinkle extra shredded mozzarella over the top. Return to the oven and bake for an additional 5-7 minutes, or until the cheese is melted and bubbly.
